Locations and Directions
West Side Location – Our west side office is located at 3253 W. North Bend Road. We share a building with other mental health professionals and there is no sign visible from street saying Collaborative Wellness Partners. We are located between and on the same side as Kroger and Lasalle High School. Please be aware we do not have admin in person at this office so you can wait in our waiting room and your provider will be out to get you. ADA accessible from rear of building. Please call upon arrival for provider to let you in the building.
Ross/Hamilton Location – This satellite office is located within Healing Springs Pharmacy. The pharmacy is located on the left of the building. You will walk in the entrance of the pharmacy. Once you are in the pharmacy your provider will meet with you in one of the consultation rooms. When you arrive you may wait for your provider in one of the chairs near the pharmacy counter and your provider will be out to get you when they are ready. There is no admin or receptionist on site at this location. This location is on 27 in the Kettering Health Medical Office building. The hours at this location are limited at this time.
